  • See subtypes: clear cell, endometrioid, mesonephric, mucinous
  • Primary nondiethylstilbesterol (DES) associated adenocarcinoma of the vagina without any morphological characteristic features of other specific types (such as clear cell, endometrioid, mesonephric, mucinous)

Pathophysiology
  • After hysterectomy, regenerative tissue responses including enhanced production of growth factors and excessive cellular turnover may provide conditions and a site for carcinogenesis
  • Rarely linked to HPV infection (Int J Gynecol Pathol 2013;32:606)
Clinical features
  • Vaginal bleeding (most common)
  • Pelvic pain
Diagnosis
  • Based on microscopic features (see below); must exclude exposure to antenatal DES, estrogen replacement therapy, metastasis from other sites, direct extension from other female genital organs

Gross description
  • Polypoid, fungating, exophytic solid mass, 2.0 cm to 5.5 cm
Microscopic (histologic) description
  • Invasive glands with cellular and architectural atypia with cuboidal or columnar tumor cells arranged in glandular structures with no specific features such as clear cells, goblet cells, mucinous cells, foci of endometriosis or mesonephric remnants
  • Can be well, moderate and poorly differentiated
